How do I know if I qualify for a medical marijuana card?
Generally, if you live in Florida and are suffering from a debilitating illness or chronic condition, you have an excellent chance to qualify for a medical marijuana card. First, you must make an appointment with Dr. Giuliano so you can be examined to see if you meet the conditions to qualify for this program. Once your condition or illness is confirmed, our experienced staff will walk you through the additional paperwork that must be submitted to the Florida Department of Health. When that paperwork is approved, you will then be given access to the state’s system of cannabis dispensaries. You need to recertify this certification every 210 days, which is just over six months.
Will I be able to pick the type of Medical Marijuana to apply for when I come to Maki Wellness Center?
At this time, you can either receive Low-THC Cannabis or Medical Cannabis. Dr. Giuilano will determine which is best for your condition. You will not be prescribed either form of medical marijuana if we do not think it will help alleviate symptoms associated with your current illness or disease.
Is there a list of conditions or chronic illnesses that currently qualify for a Medical Marijuana Card in Florida?

My condition is not listed above. Does that mean there is no way for me to qualify for a Medical Marijuana Card?
While it is preferred you have a condition above when filling out the paperwork, you may still qualify for this program. A medical marijuana card can be issued to someone suffering from “other debilitating conditions of like, kind, or class to be determined.” An evaluation at Maki Wellness Center is critical to make this determination.
When I make an appointment at Maki Wellness Center for my Medical Marijuana Card appointment, what should I bring with me?
When you come to our facility for your appointment, please bring any medical records with you that documents your illness or condition.
